General Public: Call 367-DOCS for a list of community flu clinics.

Because of continued difficulty with managing vaccine supply and changes to insurance plans, Saint Alphonsus Corporate Health Services will not be providing flu shot clinics except to employers that plan to pay for the vaccine for their workforce.

*Please follow the steps listed below to schedule your corporate flu clinic.

Vaccine pricing will be $25.00 per dose this year. A minimum of 25 participants is needed.

Flu Shot Clinic Registration Form

Complete the flu shot clinic registration form. Once the form is submitted, you will be contacted by a scheduler within three (3) business days. We are limited in our supply and our nursing resources this year. We will do our best to accommodate all requests.
