Advance Directives

Each day as an adult, you make many decisions. From the very simple to the more complex, you make choices about your home, your career, your life. You plan ahead for special occasions, and life events you expect to happen, like vacations, weddings and births.

An important choice you do not want to overlook is your choice for healthcare in the future. One way to ensure your wishes are known and followed is to establish Advance Directives. Advance Directives help you in determining the kind of care you want to receive and recording your wishes in writing.

Advance Directives will help you to make your wishes known to your physician, to your family and loved ones. They can give you peace of mind that your wishes will be honored and make difficult decisions easier for your loved ones.

Advance Directives in healthcare include a Living Will and Durable Power of Attorney. A Living Will is a document that directs your physician and family to the type of life-prolonging treatments or procedures you want to receive. A Durable Power of Attorney for Healthcare allows you to name another person to make medical decisions on your behalf at any time that you are unable to make them for yourself.

The Idaho Secretary of State office maintains an online file of Advance Directives which can be very helpful to your physician, family and emergency teams in the event you are unable to let your wishes be known. Information on the Idaho State Advance Directive registry can be obtained at www.sos.idaho.gov/general/hcdr.htm.
